Once a participant joins the Scheme, a support planner will be allocated to them. The support planner will work closely with the participant, their key support people (if needed) and service providers to develop a personal support plan.
The support plan will be tailored to the participant’s goals and needs and will take into consideration their unique and personal circumstances. It places the participant at the centre of the decision-making process, maximising their independence and dignity.
The support planner will:
- Work with the participant and service providers to facilitate access to the services the participant needs to achieve their goals, based on their personal support plan
- Meet regularly with the participant to review their support plan
- Identify any new goals and supports required.
