SMART goal cards

The NIISQ Agency has developed SMART goal cards to assist in creating meaningful and relevant goals post-injury. SMART goals can be a useful tool in the support planning and service providing process funded by NIISQ.

SMART goal-setting principles can help you set meaningful goals. Our SMART goal cards were co-designed with NIISQ participants and created using data-driven insights.
SMART stands for:
- Specific
- Measurable
- Achievable
- Relevant
- Timebound.

Before your scheduled MyPlanning session you can review the cards and set aside the ones you wish to discuss. This way, you can guide the conversation and explore topics and goals that matter to you. It can also be helpful to go through the deck with your family or people who support you and identify areas they think may be important for you to focus on too.
During your MyPlanning session, your Support Planner may bring a deck of SMART goal cards. They can use these cards to prompt ideas or delve deeper into the goals you want to set. It helps them frame your goals into a SMART format, ensuring you get the most out of the support available to you.
